A case report of niacin in the treatment of restless legs syndrome

A woman, age 87years, with no other remarkable medical history other than allergies, has suffered from restless legs syndrome (RLS) of many decades duration. She would awaken at night approximately every 45min or so, and would need to get out of bed and walk around in order to relieve her symptoms. At times she found that drinking coffee would give some relief. Her physician had prescribed Mirapex (pramipexole dihydrochloride), the leading medication for this disorder, and while it gave her some relief, especially initially, it led to severe morning fatigue and grogginess. In this study, we explored which is the major player in nitroglycerin biotransformation. In vivo, rats was treated with nitroglycerin, the blood pressure and plasma calcitonin gene – related peptide (CGRP) were measured. The inhibitor of GST (ethacrynic acid) or ALDH-2 (cyanamide) was given before nitroglycerin treatment; In vitro, the isolated aorta rings were incubated with nitroglycerin to obtain the concentration – response curve. Ethacrynic acid or cyanamide was pre-incubated with the rings before nitroglycerin treatment. The release of CGRP from the aorta rings was determined. Both ethacrynic acid and cyanamide were able to reverse the depressant action of nitroglycerin while the inhibitory effect of cyanamide was more profound. Authors: Hegde RN, Shetti NP, Nandibewoor ST A simple and rapid electrochemical method was developed for the determination of trace-level trazodone, based on the excellent properties of multi-walled carbon nanotubes (MWCNTs). The MWCNT-modified glassy carbon electrode was constructed and the electrochemical behavior of trazodone was investigated in detail. The cyclic voltammetric results indicate that MWCNT-modified glassy carbon electrode can remarkably enhance electrocatalytic activity towards the oxidation of trazodone in neutral solutions. It leads to a considerable improvement of the anodic peak current for trazodone, and allows the development of a highly sensitive voltammetric sensor for the determination of trazodone. Conclusions: The Fontan completion induces significant changes in the levels of antidiuretic hormone, aldosteron and renin. Prolonged drainage correlates significantly with elevated levels of aldosteron, renin and antidiuretic hormone postoperatively, and with longer bypass time, but is not influenced by lisinopril.
