Generalized anxiety disorder (GAD) is a prevalent and impairing disorder, associated with extensive psychiatric and medical comorbidity and usually characterized by a chronic course. Cork University Hospital becomes smoke-free zone

Acute Retinal Necrosis: An Uncommon Ocular Complication of the Herpes Virus

Background: Acute retinal necrosis (ARN) is a sight-threatening condition that results from the reactivation of a dormant herpes virus. This typically occurs in immunocompetent individuals ages 20-50. Initial complaints may include pain, decreased vision, and floaters. Clinical presentation includes focal peripheral areas of retinal necrosis that progress rapidly, occlusive vasculopathy and a marked inflammatory reaction in the vitreous and anterior chamber. Retinal detachment occurs in approximately 75% of patients and is a major cause of vision loss. Thirty-six percent of cases are bilateral, and only 30% of patients achieve greater than 20/200 vision. Cigarette smokers frequently describe the anxiolytic and antidepressant effects of smoking, but evidence suggests that cigarette smoking may itself increase negative affect, so that the causal direction of this association remains unclear. Although increasingly sophisticated analyses of epidemiological data may help to answer this question, observational data can never unequivocally provide evidence of causation. Here we discuss the potential utility of genetic information in determining the causal basis of the relationship of cigarette smoking and depression. (Source: The British Journal of Psychiatry)
